Job Description

Inpatient Care Coordination

As an Intensive Care Nurse Specialist at Prisma Health, you will be responsible for providing high-quality, patient-centered care to critically ill patients in our stepdown unit. You will work closely with our interdisciplinary team to coordinate care, manage complex medical conditions, and ensure seamless transitions between levels of care.

You will assess patients' physical and emotional needs, develop individualized plans of care, and implement evidence-based interventions to promote optimal outcomes. Your expertise will also include educating patients and families about their conditions, medications, and self-care techniques.

Required Skills and Qualifications

To succeed in this role, you must have:

  • A current RN license in South Carolina
  • At least one year of experience as a registered nurse, preferably in an acute care setting
  • Excellent communication and interpersonal skills
  • A strong understanding of nursing theories, research, and best practices
  • Ability to prioritize tasks, manage multiple responsibilities, and maintain accuracy and attention to detail
