Nurse Practitioner Physician Employment Opportunity in Wisconsin

We are seeking an experienced Physician Assistant or Nurse Practitioner to join our practice full time in Madison, WI. The ideal candidate will have a minimum of 3 to 5 years of acute inpatient hospital experience and be able to work autonomously. The position involves managing inpatients at a brand new LTACH in the Madison, WI area, with a focus on trach/vent/complicated wound and multiple co-morbidities. The candidate must be comfortable managing acute changes in place and have expertise in primary care, acute care, and chronic care practices.
Responsibilities:
- Conduct history and physical exams, examining patients and their medical records, pertinent documents/imaging/labs/tests
- Order and study diagnostic tests, including blood tests, X-rays, etc.
- Diagnose, manage, and treat acute episodic illnesses and stable chronic conditions, evaluate current therapies, health maintenance, health promotion, illness prevention education, and counseling, and manage urgent care matters
- Prescribe medications and write referrals as needed
- Maintain accurate records and schedules, establishing and maintaining a system for tracking and coordinating the designated patient caseload and their progress throughout the entire acute or chronic episode of care
- Inform patients about their health conditions and prescribed medications, and educate patients and their family members on diseases, epidemics, and prevention
- Collaborate with the healthcare team, monitoring, evaluating, and communicating relevant data regarding clinical problems to Physician, as appropriate, and members of the interdisciplinary team
- Oversee patients' overall health, identifying opportunities for each patient's needs relative to his/her age, and monitors progress of patients and collaborates with other team members to ensure continuity of care
- Promote good health practices and identifies and addresses potential deterrents to compliance with treatment regime
- Advance professional knowledge and conduct research, and follows up with patients if they are discharged to assess if needs are being met and determines if additional follow-up care is required
Requirements:
- Minimum 3 to 5 years of acute inpatient hospital experience as a physician assistant or a nurse practitioner
- Expertise in primary care, acute care, and chronic care practices
- Valid nurse practitioner or physician assistant license in WI
- Master's degree (graduation from an accredited NP or PA program)
- ACLS Certified
