Clinical Coordinator RN 3-11

We are seeking a highly skilled and compassionate Clinical Coordinator RN 3-11 to join our team at UHS. As a key member of our clinical team, you will be responsible for receiving inquiry calls, assessing patient needs, and coordinating the clinical team effort to ensure that these needs are met.
Key Responsibilities- Receive and respond to referral/inquiry calls, providing information on programs and services and coordinating assessments or tours of our residential facility.
- Perform assessments and medical screenings, collaborating with clinical referral sources, families, doctors, agencies, and other callers to ensure smooth coordination of assessments or admissions.
- Aid referral sources, families, or callers in triage to services in the community when our facility does not meet the assessed or stated needs.
- Inform voluntary patients of their legal rights in language they can understand prior to admission, and inform involuntary patients of their legal rights within 24 hours of admission.
- Collaborate with Business Office staff to ensure payer information is obtained and verified, and that financial information is provided to patients.
- Complete administrative paperwork, including consent and releases, to ensure patients are fully informed during the admission process.
- Expertly manage MS4/Midas and ensure 100% entry of all calls and admissions, providing director with statistical reports and suggesting measures to improve care delivery and utilization.
- Compile and maintain community resources for outside referrals to all levels of care, including support groups, acute and residential facilities, and resources for clients with limited resources.
We are seeking candidates with a Master's Degree in a mental health field, plus 1 year of experience, or a graduate of an accredited nursing school. A Texas license as an L.P.C, R.N., or LMSW is required for acute admissions coordinators. 2 years of mental health experience is also preferred.
