Job Description

Job Title: Mental Health Service Coordinator

Job Summary: We are seeking a highly skilled Mental Health Service Coordinator to join our team at Barrier Free Living. The successful candidate will be responsible for providing mental health services to our clients, including individual and group counseling, case management, and crisis intervention.

Key Responsibilities:

  • Caseload Management: Hold a caseload of 10-15 clients and provide regular check-ins to ensure their mental health needs are being met.
  • Assessments and Service Plans: Complete program intake/assessment of new clients and develop service plans that are mutually developed with each client.
  • Counseling and Support: Provide individual supportive counseling with clients to assist in developing skills to stabilize their medical, psychosocial, and behavioral health status.
  • Crisis Intervention: Provide crisis intervention and other services to assist clients in developing better coping skills.
  • Interdisciplinary Collaboration: Collaborate with the interdisciplinary team to obtain strategies to assist clients in achieving identified goals to maintain stability.
  • Advocacy: Accompany clients to Family/Criminal/Housing Court hearings and advocate for clients as needed.
  • Referrals and Support: Provide referrals for legal assistance and any other service provider within the community as needed.

Requirements:

  • Master's Degree: Master's degree in social work, psychology, counseling, or related field.
  • Experience: Experience in supportive counseling, harm reduction stages of change model, motivational interviewing, and trauma-informed care.
  • Skills: Excellent communication and interpersonal skills, with the ability to work effectively with clients, staff, and community partners.
