GHR Healthcare is seeking a skilled Nurse Practitioner to join our team in providing exceptional care to patients in their homes.
This is a full-time position, Monday to Friday, 8:30 AM - 4:30 PM, with some flexibility. The ideal candidate will have experience in home visits, patient care management, and symptom assessment.
Key Responsibilities:
- Conduct home visits and provide patient care, addressing symptoms and managing patient needs.
- Provide referrals, orders, and coordinate with specialists as needed.
- Conduct health assessments and diagnose and treat patients who require management of symptoms.
Benefits:
- Competitive base salary: $110,000 - $135,000 per year.
- Quarterly bonuses up to $3,125.
- Comprehensive medical, dental, and vision insurance.
- 403b Plan with company match.
- Generous PTO.
- Mileage reimbursement ($0.67 per mile).
Requirements:
- Current, unrestricted NP license.
- Palliative Care, Hospice, Geriatrics, or Long-Term Care experience preferred.
- Ability to obtain DEA licensure.
- Minimum 1 year of NP experience.
