Job Summary

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team at Amedisys. As a Hospice Nurse, you will provide patient-centered care to individuals in their homes, focusing on comfort, dignity, and quality of life.

Responsibilities
  • Perform initial nursing assessments on new admissions
  • Develop, implement, and revise individualized patient care plans
  • Regularly communicate patient progress to the clinical manager, physician, and care team
  • Educate families on patient condition, needs, and care
  • Make referrals to other hospice care disciplines as needed
  • Perform on-call responsibilities and services to patients/families as assigned
Qualifications
  • Current RN license, specific to the state(s) you are assigned to work
  • One year of nursing experience preferred
  • Current CPR certification
  • Valid driver's license, reliable transportation, and liability insurance
