Job Summary

We are seeking a highly skilled LVN Care Coordinator to join our team at Central Valley Health Plan. As a key member of our operations team, you will play a critical role in ensuring timely care coordination for our patients.

Key Responsibilities
  • Outreach and scheduling patients for annual wellness visits
  • Liaise between RN Case Managers, Medical Directors, patients, physicians, community resources, and other healthcare providers
  • Ensure members and providers are educated on annual wellness visit requirements and health care gaps identified by the CMS STARs scorecard
  • Collaborate with RN Case Managers to develop and implement care plans
  • Provide exceptional customer service and communication skills
Requirements
  • High School Diploma or equivalent required; Associates degree preferred
  • Graduate of an accredited Licensed Vocational Nurse (LVN) program required
  • Current licensure as a Licensed Vocational Nurse (LVN) in the state of California required
  • Current American Heart Association (AHA) Healthcare Provider CPR card required
  • Two (2) years of healthcare experience in an acute care setting preferred
Preferred Qualifications
  • Excellent communication, critical thinking, and problem-solving skills
  • Competent organizational and planning skills
  • Ability to work under pressure and balance evolving priorities
  • Familiarity with standard desktop and windows-based computer systems
  • Knowledge of hospital protocol and procedures, clinical standards, and outcomes
