  • Must be a current CRNP license holder in Pennsylvania
  • 2-3 years of patient care experience
  • Excellent clinical skills and knowledge of agency policies, federal and state regulations, and third-party payers
  • Solid computer skills and excellent organizational and communication skills

Key responsibilities include:

  • Providing skilled nursing care, including teaching, treatments, and health promotion practices
  • Executing preventive, rehabilitative, and restorative nursing procedures
  • Assessing patient signs and symptoms, reporting reactions to treatments, and changes in patient condition
  • Developing and revising patient plans of care collaboratively with physicians, patients, and designated representatives
  • Supervising and evaluating care provided by Licensed Practical Nurses and home health aides
  • Coordinating patient care, serving as the primary nurse for designated patients, and evaluating care effectiveness
  • Maintaining confidentiality, reporting compliance concerns, and adhering to high ethical standards

Additional requirements include reliable transportation and familiarity with communication channels for problems or concerns.
