Job Summary

We are seeking a skilled Case Manager to join our team at Centers for Independence. As a Case Manager, you will be responsible for providing comprehensive care and support to adults living with mental illness.

Key Responsibilities
  • Develop and implement treatment plans in collaboration with clients and healthcare professionals.
  • Provide psychosocial rehabilitation, symptom monitoring, and supportive counseling to clients.
  • Monitor clients' psychiatric medication regimes and ensure compliance.
  • Provide crisis intervention and supportive counseling to clients in crisis.
  • Coordinate access to community resources, day programs, and outside healthcare providers.
  • Assist clients with financial planning and development of income resources.
  • Coordinate consumer access to entitlement programs and act as protective payee for Social Security benefits when necessary.
  • Provide treatment-related education and liaison with clients' families and natural supports.
  • Assist clients in securing adequate housing and prompting and training for skills necessary to maintain housing.
  • Coordinate the efforts of all professionals and paraprofessionals involved with each client.
  • Formulate appropriate discharge plans with assistance from clients and program supervisors.
  • Accurately document and maintain case files as required by agency policy, government regulations, and applicable local, state, and federal codes.
Requirements
  • Bachelor's Degree or Master's Degree in Social Work, Counseling, or a related Human Services field.
  • Minimum 1,000 hours of post-graduate experience working with adults with severe and persistent mental illness (preferable).
  • Valid driver's license with reliable transportation and vehicle insurance that meets agency standards.
Working Conditions

This is a medium work position that requires exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.

The employee may be required to perform other job-related duties as requested by the designated work leader(s). All requirements are subject to change over time.
