We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice Case Manager. In this role, you will provide direct care and coordinate with our interdisciplinary team to ensure our patients receive the highest quality care.
Responsibilities- Conduct comprehensive assessments of patients and families to identify their unique needs and develop personalized care plans.
- Collaborate with our healthcare team to ensure seamless care coordination and effective communication.
- Administer medication and treatment as prescribed by our physicians, and provide education to patients and families on their care plans.
- Document patient progress and maintain accurate clinical records.
- Participate in interdisciplinary team meetings to discuss patient care and address any concerns.
- Graduate of an accredited nursing program with current licensure to practice in the state of operation.
- At least one year of nursing experience, with three or more years preferred.
- Experience in hospice or a similar setting is a plus.
