About the Role

We are seeking a highly organized and detail-oriented Patient Services Coordinator to join our team at CenterWell Home Health. As a key member of our care coordination team, you will play a vital role in ensuring the seamless delivery of high-quality patient care.

Key Responsibilities
  • Manage patient schedules and coordinate with field staff, patients, and physicians to maintain continuity of care.
  • Assist with day-to-day office and staff management, including scheduling, communication, and administrative tasks.
  • Initiate infection control forms as needed and ensure timely completion and submission.
  • Provide backup support during busy periods, including receiving calls from field staff and assisting with weekly case conferences.
  • Maintain accurate records, including client hospitalization logs and coordination notes.
  • Process scheduling requests, including rescheduling declined or missed visits and ensuring supervisory visits are scheduled.
  • Run scheduling reports, including Agent Summary Report and Missed Visits Done on Paper Report.
  • Prepare weekly agent schedules and perform initial review for productivity and geographic issues.
Requirements
  • 1+ year of medical office experience, with Excel experience a plus.
  • Excellent communication and organizational skills, with the ability to manage multiple assignments.
  • Coding certification is preferred.
  • Valid state driver's license and automobile liability insurance.
