Community Resource Specialist

We are seeking a skilled Community Resource Specialist to join our team at Sciometrix. In this role, you will play a critical part in assessing community resources and connecting individuals and families with the support they need to thrive.
Key Responsibilities- Community Resource Assessment: Assess the availability, accessibility, and effectiveness of community resources, including healthcare facilities, social service agencies, educational programs, employment services, housing options, and support groups.
- Client Needs Assessment: Conduct comprehensive assessments of individual clients or families to identify strengths, challenges, and goals, and determine which community resources are most beneficial for addressing their needs.
- Referral and Coordination: Make referrals to appropriate community resources and services that align with clients' needs and preferences, and facilitate connections between clients and service providers.
To be successful in this role, you will need:
- Strong assessment and referral skills;
- A deep understanding of community resources and services;
- Excellent communication and interpersonal skills;
- The ability to work effectively with diverse populations and stakeholders;
- Bachelor's degree in Social Work or a related field.
