Job Summary

We are seeking a skilled Infusion Therapy Liaison to join our team at Providence Home Services Oregon. As a key member of our team, you will provide direct nursing services related to infusion therapy for patients in various settings, including hospitals, homes, clinics, and alternate living sites.

Key Responsibilities:

  • Provide education to patients discharging from area hospitals, focusing on infusion therapy and related care.
  • Collaborate with various providers, patients, caregivers, physicians, and payors to ensure seamless care coordination.
  • Deliver infusion nursing care in the home or clinic, working closely with patients and their families to achieve optimal outcomes.
  • Develop and maintain relationships with healthcare providers, payors, and community resources to ensure comprehensive care.

Requirements:

  • Education to meet certification, license, or registration requirements.
  • Oregon Registered Nurse License upon hire.
  • National Provider BLS - American Heart Association within 30 days of hire.
  • Must be licensed in states wherever care is delivered upon hire.

Preferred Qualifications:

  • 1 year of experience with acute hospital care, infusion, or home health.
