About the Role

We are seeking a skilled Infusion Therapy Liaison to join our team at Providence Home Services Oregon. As a key member of our nursing staff, you will provide direct nursing services related to infusion therapy for patients in various settings, including hospitals, homes, clinics, and alternate living sites.

Your primary focus will be to provide education to patients discharging from area hospitals, ensuring they receive the necessary support and care to manage their infusion therapy at home. You will also provide infusion nursing care in the home or clinic, working collaboratively with various providers, patients, caregivers, physicians, and payors to ensure seamless care coordination.

Responsibilities
  • Provide direct nursing services related to infusion therapy for patients in various settings.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with healthcare teams to ensure comprehensive care coordination and communication.
  • Provide education and support to patients and their families to ensure successful infusion therapy management at home.
  • Conduct regular assessments and evaluations to ensure patient safety and optimal outcomes.
Requirements
  • Current Oregon Registered Nurse License or Washington Registered Nurse License (Vendor Managed) or National Registered Nurse Compact License.
  • National Provider BLS - American Heart Association certification within 30 days of hire.
  • Unencumbered Registered Nurse License in state(s) of hire, and wherever care is delivered.
Preferred Qualifications
  • 1 year of experience with acute hospital care, infusion, or home health.
