Responsibilities
  • Manage and oversee clinical care for residents in skilled nursing facilities (SNFs).
  • Conduct daily rounds and prescribe necessary medications and treatments.
  • Deliver high-quality, patient-centered primary care services.
  • Ensure timely documentation of all patient interactions.
  • Be available for consultations with nursing staff via phone or secure messaging.
  • Utilize Theoria's proprietary electronic health record (EHR) system, ChartEasy, for documentation.
  • Familiarity with common facility EHR systems such as PointClickCare, MatrixCare, and American HealthTech is beneficial.
  • Arrive on-site punctually for scheduled shifts.
  • Reliable transportation is essential.

Work Schedule
  • Rounding occurs from 7 AM to 1 PM local time, during which the provider must be accessible to nursing staff for urgent matters.
  • Shifts typically commence around 7 AM local time, with reasonable flexibility.
  • Workdays are Monday through Friday.
  • Charting and documentation can be performed off-site.
  • This role requires in-person attendance.
