Clinical Triage Nurse - Dermatology - Full Time

Position Overview:
The Clinical Triage Nurse will deliver essential triage services via telephone to support adherence to patient care plans and swiftly identify individuals with critical health conditions. This role involves evaluating and prioritizing patient needs through assessments, monitoring, and documenting changes in their health status. Triage Nurses play a crucial role in all facets of patient health, requiring precision and thoroughness in maintaining detailed assessments and reports to communicate effectively with healthcare providers and patients alike. Responsibilities include preparing, assisting, and educating patients for examinations and treatments, as well as informing family members on relevant health education topics.

Key Responsibilities:
Core Duties

Telephone and In-Person Triage
1. Conducts telephone triage to assess patient needs by documenting and communicating patient conditions to healthcare providers and advising patients based on provider directives.

  • Responds to patient calls within a designated timeframe based on medical urgency.
  • Accurately documents and reports patient conditions in compliance with departmental policies.
  • Adheres strictly to established practice protocols.
  • Consults patient records during interactions.
  • Returns messages according to established guidelines.
  • Communicates test results following protocol.
2. Provides education to patients and families to ensure understanding and awareness, recommending community resources as necessary.
  • Instructs patients as directed by healthcare providers.
3. Medical Home Coordination
  • Records all laboratory tests, procedures, and appointments.
  • Follows up on missed appointments and procedures.
  • Supports Health Coaches by managing patient lists and scheduling as instructed.
4. Maintains a clean and organized work environment to promote health and safety.
  • Ensures a tidy workspace in accordance with departmental standards.
5. Manages prescription renewals and inquiries to enhance patient care.
  • Processes prescription refills in line with established protocols.
  • Completes pharmacy requests accurately.
6. Delivers age-appropriate care to ensure patient comprehension and comfort.
7. Conducts Nursing Assessments
  • Utilizes clinical judgment to formulate care plans.
  • Evaluates the severity of patient conditions and prioritizes interventions.
  • Documents interactions clearly using standardized formats.
  • Facilitates communication between patients and providers.
  • Conducts educational sessions for patients and families, documenting understanding.
  • Coordinates referrals to specialists for continued care.
8. Prepares examination rooms and necessary equipment for patient evaluations.
  • Sets up medical instruments and supplies according to provider guidelines.
9. Prepares patients for examinations by reviewing medical histories and checking vital signs.
  • Follows protocols for patient preparation.
10. Assists healthcare providers in facilitating patient care by retrieving necessary medical records and lab work.
  • Obtains requested tests and documentation as directed.
11. Administers medications under the guidance of a provider, ensuring patients and families understand treatment protocols.
12. Assists with phlebotomy, ensuring proper specimen handling and documentation.
  • Follows clinic policies for specimen collection.
13. Ensures accurate order entry and documentation without errors.

Qualifications:

  • Education
    • Graduation from an accredited nursing program (RN) is required.
  • Experience
    • At least one year of clinical experience is required.
  • Licenses/Certifications
    • Current Basic Life Support (BLS) certification is preferred at the time of hire, required within three months.
    • A valid Registered Nurse (RN) license is required.
  • Skills/Knowledge/Abilities
    • Understanding of medical terminology and procedures is essential.
    • Ability to recognize and diagnose various health conditions.
    • Effective time management and task prioritization skills.
    • Proficient in computer data entry.
    • Strong interpersonal skills for patient interactions.
    • Ability to listen critically and convey information accurately.
    • Proactive problem-solving skills.
    • Ability to learn and navigate electronic health record systems.
    • Excellent verbal communication skills.
    • Ability to follow written and verbal instructions.
    • Understanding of HIPAA regulations.

Physical Requirements:

  • Medium Work - Ability to exert up to 50 pounds of force.
