Core Duties
Telephone and In-Person Triage
1. Conducts telephone triage to ascertain patient needs by documenting and conveying patient conditions to healthcare providers and advising patients based on provider guidelines.
- Responds to patient calls within established timeframes based on medical urgency.
- Accurately logs and communicates patient conditions in line with departmental protocols.
- Follows established practice protocols diligently.
- Consults patient records during interactions.
- Returns messages in accordance with established protocols.
- Provides test results following departmental guidelines.
- Educates patients as per provider instructions.
- Documents all lab results, procedures, and appointments accurately.
- Completes follow-up on missed appointments and procedures.
- Assists health coaches in managing patient lists and scheduling appointments.
- Ensures work areas are maintained according to departmental standards.
- Handles prescription requests in accordance with departmental protocols.
- Completes pharmacy requests accurately.
- Utilizes nursing judgment to formulate care plans.
- Evaluates the severity of patient conditions and prioritizes interventions.
- Documents interactions clearly using established communication formats.
- Facilitates communication between providers and patients effectively.
- Conducts patient and family education, documenting understanding in patient records.
- Coordinates referrals to specialists for continued care.
- Ensures exam rooms and medical equipment are prepared according to provider standards.
- Follows provider protocols and company policies during patient preparation.
- Obtains requested tests and documentation as directed.
- Adheres to clinic policies during specimen collection.
Qualifications:
- Education
- Graduation from an accredited nursing program (RN) is required.
- Experience
- A minimum of one year of clinical experience is required.
- Licenses/Certifications
- Current Basic Life Support (BLS) certification is preferred at hire, required within three months.
- A valid Registered Nurse (RN) license is required.
- Skills/Knowledge/Abilities
- Understanding of medical terminology and procedures is essential.
- Knowledge of disease recognition and diagnosis is required.
- Ability to manage time effectively and prioritize tasks.
- Proficient typing skills for accurate documentation.
- Ability to provide compassionate patient care.
- Strong listening skills to convey information effectively.
- Proactive problem-solving abilities.
- Ability to navigate patient information systems proficiently.
- Excellent verbal communication skills are essential.
