We are seeking an experienced Nurse Practitioner with Endocrinology expertise to provide high-quality patient care and deliver full scope Endocrinology services in collaboration with our physicians.
Responsibilities- Review patient history, reports, and examination results to determine diagnosis and design individualized treatment plans.
- Perform physical examinations, order and interpret diagnostic tests, and evaluate patients with hormonal diseases and conditions.
- Provide comprehensive cost-effective diabetes care, including support from CDE/Certified Diabetes Educator/RD services.
- Recommend suitable management for patients, including preventive health recommendations and habits.
- Prescribe medications in accordance with New York State and federal law, formulate ongoing treatment and disease management plans, and maintain accurate medical records.
- Provide relationship-centered care, taking into account patients' psychosocial and physical needs, and communicate effectively with the clinical team to ensure total delivery of quality care.
- One to three years of endocrinology clinical experience in an outpatient setting.
- Ability to diagnose and treat a wide variety of endocrinology conditions with a focus on improving patient outcomes.
- Demonstrate competency in diagnosis and treatment to assure high standards of patient care and safety are maintained.
- Patient-centered approach toward a positive patient experience.
- Excellent communication skills.
- Interest in Population Health Management.
- Master of Science degree in Nursing (MSN).
- Graduate of a Nurse Practitioner education program registered by the New York State Education Department.
- Certification as a Nurse Practitioner from a New York State approved accrediting body - American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP).
- Current, unrestricted New York State Registered Nurse license.
- Active Drug Enforcement Agency (DEA) registration or eligibility for registration.
- Active Medicare/Medicaid Enrollment or eligibility to enroll.
- BLS or ACLS certification required.
- Strong communication skills, both oral and written.
- Comfortable with the use of Electronic Medical Record (EMR) system and Microsoft Office.
