Our clinical teams work closely with physicians and other specialists to offer advanced illness management, a specialized form of high-touch palliative care designed to provide relief from the symptoms and stress of illness while improving quality of life for our patients.
Job SummaryWe are seeking a skilled Registered Nurse to join our team as a Palliative Care RN in a hospice and home care setting. The successful candidate will work with patients, their families, and other healthcare professionals to provide and coordinate high-quality end-of-life care.
Responsibilities- Conduct in-person and telephonic assessments to palliative care patients in collaboration with the interdisciplinary team.
- Provide education to patients and families on disease process, self-care techniques, end-of-life care, medications, nutrition, and dietary needs.
- Communicate any findings that identify appropriate preventive and rehabilitative skilled nursing or therapy needs to the PCP or AIM Palliative NP.
- Coordinate care with palliative team members and communicate change in condition or signs and symptoms of decline to Palliative NP and/or PCP.
- Support hospice referral as appropriate and document timely clinical records on all patients.
- Registered Nurse license to practice in the state.
- 2 years of prior skilled Registered Nurse experience.
- Demonstrated ability to provide quality patient care.
- Nursing experience in palliative care, oncology, hospice, or geriatric care preferred.
