Job Summary

We are seeking a highly motivated and experienced Registered Nurse (RN) to join our team as a Palliative Care Nurse. As a Palliative Care Nurse, you will work closely with patients, families, and healthcare professionals to provide comprehensive care and support to individuals with advanced illnesses. This is a full-time position, Monday through Friday, 8am-5pm, with no call or weekend requirements.

Key Responsibilities
  • Conduct in-person and telephonic assessments to identify patient support needs and develop individualized care plans.
  • Provide education and support to patients and families on disease management, self-care techniques, and end-of-life care.
  • Collaborate with the interdisciplinary team to coordinate care and ensure seamless transitions.
  • Document timely clinical records and maintain accurate patient information.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements
  • Current RN license to practice in the state.
  • 2 years of prior skilled Registered Nurse experience.
  • Demonstrated ability to provide quality patient care and work effectively in a team environment.
  • Strong communication and interpersonal skills.
  • Ability to work independently and make sound critical decisions.
