Job SummaryWe're seeking a compassionate and skilled Registered Nurse to join our hospice care team. As a RN on our team, you'll play a vital role in evaluating patients, creating care plans, and communicating with patients, families, and the care team.
Responsibilities- Evaluate patients and create personalized care plans
- Communicate effectively with patients, families, and the care team
- Provide high-quality care and support to patients and families
- Collaborate with the care team to ensure seamless care delivery
- Graduate of an accredited school of nursing with a current state license as a registered nurse
- 2+ years of nursing experience in a clinical care setting (hospice experience preferred)
- Ability to sit, stand, bend, move intermittently, and lift at least 25 lbs
