Nurse Care Manager

We are seeking a skilled Nurse Care Manager to join our team at Neighborhood Service Organization. As a Nurse Care Manager, you will be responsible for conducting comprehensive assessments of patient biomedical, psychological, social, and functional needs. You will engage patients as part of a care team through advocacy, ongoing communication, health education, identification of resources, and service facilitation.
Key Responsibilities:
- Conduct comprehensive assessments of patient biomedical, psychological, social, and functional needs.
- Engage patients as part of a care team through advocacy, ongoing communication, health education, identification of resources, and service facilitation.
- Utilize professional judgment, critical thinking, motivational interviewing, and self-management techniques to assist patients in overcoming barriers to goal achievement.
- Collaborate with appropriate physicians, clinical pharmacists, social workers, nurses, therapists, and community health workers to co-manage patients with complex medical and social needs.
- Perform outbound contacts to patients approved by the multidisciplinary team.
- Document services and outcomes in patient plan of care. Participate in weekly meetings to address patient outcomes.
- Participate in the quality improvement and evaluation process with activities designed to reduce risk, inclusive of data collection, analysis, and follow-up interventions.
- Adhere to the SWCS code of ethics and comply with local, state, and federal laws and the mental health codes.
- Serve on agency committees, working groups, and other projects as assigned.
Requirements:
- Associate Degree in Nursing (ADN), Bachelor of Science in Nursing (BSN), or diploma from an accredited and approved nursing program.
- Two (2) years of nursing experience.
- Case Management experience preferable.
- Registered Nurse or Licensed Practical Nurse with a valid, unrestricted State of Michigan license.
- Possession of a valid driver's license and access to a private vehicle for day-to-day job performance.
Preferred Qualifications:
- Computer skills and knowledge including Windows, Microsoft Word, and Microsoft Excel.
- Ability to communicate effectively in writing and verbally.
- Demonstrate the ability to triage and apply critical thinking skills.
- Understanding of community resources and the needs of diverse populations.
