Job Summary

The Crossover Nurse is a critical member of our team, responsible for creating a strong patient experience. This role actively manages patient care by coordinating and connecting patients with specialists and their facilities. The ideal candidate will follow up with patients, guide them through the process, and embody Crossover's values in a fast-paced and rapidly changing environment.

Key Responsibilities
  • Patient Care Coordination: Deliver patient care, practicing under the oversight of an RN or clinic supervisor.
  • Collaboration and Communication: Collaborate with patients and care teams to facilitate a patient-centered medical home.
  • Evidence-Based Nursing: Provide evidence-based nursing care at the highest community standard.
  • Specialist Coordination: Coordinate and connect patients with specialists and their facilities.
  • Phlebotomy and Immunization Services: Provide phlebotomy services and immunization services.
  • Additional Duties: Perform other duties as assigned.
Required Qualifications
  • Licensed Vocational Nurse: Licensed Vocational Nurse from an accredited school.
  • Clinical Experience: A minimum of 3 years of clinical experience.
  • Certifications: Current BLS (Basic Life Support) certification.

  • Fast-Paced Environment: Experience in a fast-paced and rapidly changing environment.
  • Strong Work Ethic: Demonstrated strong work ethic and takes pride in a detailed-oriented approach to work.
  • Organizational Skills: Highly organized and enjoys managing projects, big and small, to completion.
  • Creativity and Flexibility: Creative, flexible, and navigates through obstacles.
  • Execution Skills: Excellent follow-through with execution skills.
  • Problem-Solving: Able to handle ambiguity and solving issues.
Physical Job Requirements
  • Physical Demands: Requires standing, walking, and sitting for extended amounts of time.
  • Lifting and Carrying: Occasionally lift and carry items weighing up to 50 lbs.
  • Manual Dexterity: Manual and finger dexterity and hand-eye coordination.
  • Full Range of Motion: Includes full range of body motion, including potential handling and lifting patients.
  • Vision and Hearing: Requires corrected vision, hearing, and speech within normal ranges.
