Job Overview

Choice Matters Home Healthcare is seeking a dedicated and experienced Home Health LPN to deliver high-quality skilled nursing care to patients in their homes. As a Home Health LPN, you will play a pivotal role in ensuring the highest level of patient care and achieving optimal outcomes.

Responsibilities
  • Administer prescribed medications and treatments as per physician's orders
  • Assist with wound care, catheter care, and other specialized procedures
  • Monitor and document patient progress, maintaining accurate and up-to-date records
  • Educate patients and their families on health management, medications, and treatment plans
  • Collaborate with registered nurses, therapists, and other healthcare professionals to ensure seamless care coordination
  • Communicate effectively with physicians and other members of the healthcare team
  • Adhere to all regulatory guidelines and maintain patient confidentiality
Requirements
  • Active Licensed Practical Nurse (LPN) license in the state of FL
  • Minimum of 1-2 years of LPN experience; home health experience is preferred
  • Strong clinical skills and the ability to provide compassionate patient-centered care
  • Proficiency in administering medications and performing nursing procedures
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Comfortable using electronic health records and other technology tools
