Role Summary:
The Resident Assessment Coordinator will oversee the Resident Assessment Instrument (RAI)/Minimum Data Set (MDS) processes within our facility. This role involves the meticulous review and management of MDS assessments in alignment with regulatory standards.
Key Responsibilities:
Your significant contributions will encompass:
- Coordinating the formulation and updates of resident Interdisciplinary Care Plans based on RAI/MDS data.
- Collaborating with nursing staff and other professionals to ensure precise care plan objectives.
- Facilitating effective communication across various disciplines to ensure timely completion and submission of RAI/MDS documentation.
- Generating accurate reports on Quality Measures/Quality Indicators (QM/QI) derived from RAI/MDS information.
- Actively participating in Quality Improvement initiatives.
- Implementing educational strategies to address evolving program or service requirements.
