Home Care RN Opportunity with Almost Family

We are seeking a dedicated and compassionate Registered Nurse to join our team in providing high-quality home care services to patients in Maribel, WI and surrounding areas.

As a Registered Nurse with Almost Family, you will have the opportunity to work in a dynamic and supportive environment, delivering personalized care to patients in the comfort of their own homes.

Key Responsibilities:

  • Provide high-quality clinical services to patients in their homes, in accordance with the plan of care and within infection control standards.
  • Conduct thorough clinical nursing assessments to ensure patient eligibility and medical necessity for services.
  • Develop and revise individualized plans of care and service plans according to federal and state program requirements.
  • Collaborate with other healthcare professionals to achieve and maintain continuity of patient care.

Requirements:

  • Current RN licensure in the state of practice.
  • Minimum of one year experience as an RN preferred.
  • Current drivers license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification required.
