Job Summary

We are seeking a compassionate and skilled Registered Nurse to join our team in providing high-quality care to patients in their homes. As a Registered Nurse in Home and Community Based Services, you will be responsible for delivering personalized care and support to patients, ensuring their comfort, safety, and well-being.

Key Responsibilities:
  • Conduct thorough clinical assessments and develop individualized plans of care to meet the unique needs of each patient.
  • Provide skilled nursing care and interventions to patients in their homes, including wound care, medication management, and education.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Document patient progress and outcomes in accordance with agency policies and regulatory requirements.
Requirements:
  • Current RN licensure in the state of practice.
  • Minimum of one year of experience as an RN, preferably in home health care.
  • Current CPR certification.
