Job Title: Home Health Registered NurseJob Summary:

We are seeking a skilled and compassionate Home Health Registered Nurse to join our team at Cynet Health. As a Home Health Registered Nurse, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to develop and implement personalized care plans.

Responsibilities:
  • Provide comprehensive home nursing care to patients, including wound care, medication management, and education on disease management and self-care.
  • Conduct thorough assessments of patients' physical and emotional needs, developing and implementing care plans to address these needs.
  • Collaborate with our interdisciplinary team, including physicians, therapists, and social workers, to ensure comprehensive and coordinated care.
  • Supervise and evaluate the work of Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses, providing guidance and support as needed.
  • Travel to patients' homes, attending periodic office meetings as required.
  • Have reliable transportation, a current driver's license, and agency-required liability insurance.
  • Meet productivity expectations, handling an average of 5-8 patients per day.
  • Manage various types of cases, including Start of Care, Wound Care, and Follow-up Visits.
  • Perform assigned tasks covering a territory radius of up to 75 miles from the home clinic, with mileage reimbursement from patient home to patient home per IRS standard rate.
Requirements:
  • Minimum 1 full year Adult Home Health or Hospice experience required.
  • Must have license in hand.
  • CPR, BLS certifications required.
  • Proficiency in OASIS and HomeCare HomeBase (HCHB) EMR required.
  • Open to on-call rotations and traveling to locations within the assigned territory.
