Job Title: Licensed Practical Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Sturgeon Bay Health Services. As a key member of our healthcare team, you will provide high-quality patient care and support to our residents and patients.

Job Summary:

Under the supervision of the RN Unit Manager or DON, the LPN will perform as a licensed caregiver and assume responsibility and accountability for assigned residents/patients for a shift of duty. Nursing care will be provided through coordination, implementation, and evaluation of the resident's/patient's plan of care.

Responsibilities:
  • Provide direct patient care and support to residents and patients
  • Develop and implement individualized care plans
  • Coordinate and manage resident/patient care activities
  • Supervise and train nursing assistants
  • Ensure resident/patient safety and well-being
  • Follow all Center policies and procedures
Requirements:
  • Licensed in the State as an LPN
  • Current CPR certification
  • Able to use necessary supplies and equipment
  • Able to calculate drug dosage
  • Meets all health requirements imposed by law
  • Ability to relate positively, effectively, and appropriately with residents, families, community members, volunteers, and other center staff
