Nurse Care Coordinator

We are seeking a highly skilled and compassionate Nurse Navigator to join our team at Hunterdon Health. As a key member of our interdisciplinary team, you will play a vital role in coordinating patient care for individuals referred to our cancer program.
Key Responsibilities- Coordinate Patient Care: Collaborate with the healthcare team to ensure seamless transitions through the cancer care process, from referral to diagnosis and treatment.
- Patient Education: Provide patients and their families with comprehensive education on diagnosis, treatment plans, and available resources.
- Referral Coordination: Facilitate referrals to appropriate services within and outside Hunterdon Health to meet patients' physical, psychosocial, financial, and spiritual needs.
- Team Collaboration: Demonstrate effective resource management and interprofessional collaboration to ensure high-quality patient care.
- Quality Improvement: Participate in and contribute expertise to activities that strengthen our cancer program, including continuous quality improvement, relationship building, and marketing strategies.
- Professional Development: Actively seek and participate in educational opportunities to maintain knowledge and skills related to job responsibilities.
- Education: Bachelor's Degree in Nursing (or Associate's Degree with proof of enrollment in a BSN program)
- Experience: Three years of experience as a registered nurse, with at least one year of recent acute care experience or experience in a healthcare system
- Certifications: NJ state license in good standing, American Heart Association CPR certification, and certification in oncology (or completion of a qualifying patient navigation training program within one year of hire)
- Education: Bachelor's Degree in Nursing
- Experience: Five years of experience as a registered nurse, with recent experience as a care coordinator and oncology experience
- Certifications: Certified Oncology Nurse
