About the Role

We are seeking a skilled Nurse Practitioner/Physician Assistant to join our team as a Home Risk Assessment Specialist. As a key member of our clinical team, you will be responsible for conducting comprehensive health assessments in the comfort of patients' homes.


This is a PRN opportunity, allowing you to choose your schedule and work at your own pace. You will have the flexibility to work in a variety of settings, including homes, skilled facilities, and telehealth visits.


As a Home Risk Assessment Specialist, you will have the opportunity to make a meaningful impact on patients' lives by identifying potential health risks and providing personalized care recommendations.

Responsibilities

Conduct comprehensive health assessments in patients' homes, including reviewing medical history, medication adherence, social environment, and other risk factors.


Develop personalized care plans and provide education to patients and their families on healthy lifestyle choices and disease management.


Collaborate with primary care physicians, case managers, and other healthcare professionals to ensure seamless care coordination.


Utilize our proprietary Matrix Comprehensive Health Assessment tool to identify potential health risks and develop targeted interventions.


Requirements

Master's degree in Nursing or Physician Assistant studies, or equivalent experience and licensure.


Current RN and NP/PA licensure in the state of practice, with prescription authority or the ability to obtain.


Board certification by the NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty.


Minimum of 1 year experience as a Nurse Practitioner/Physician Assistant, with a strong background in clinical assessment and care management.


Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.


Ability to travel, with a valid state driver's license and adequate automobile insurance coverage.
