About the PositionAs a PRN Home Risk Assessment provider, you will be responsible for conducting comprehensive health assessments in the comfort of patients' homes. This role requires a strong clinical background, excellent communication skills, and the ability to work independently in a fast-paced environment.
Responsibilities- Conduct comprehensive health assessments in patients' homes
- Review and observe patients' current health, medical history, medication adherence, social environment, and other risks
- Develop and implement personalized care plans to address patients' unique needs
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination
- Document all assessments and care plans in accordance with Matrix Medical Network's guidelines and protocols
- Masters degree in Nursing or Physician Assistant studies
- Current RN and NP/PA licensure in the state of practice, including prescription authority
- Board certification in a Matrix-approved specialty
- Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
- Strong computer skills and familiarity with EMR software
- Excellent verbal and written communication skills
