We are seeking a dedicated Registered Nurse to join our team, providing personalized patient care in the comfort of their homes. This role offers a variety of hours and includes benefits, adaptable schedules, and competitive weekly compensation. A $1000 loyalty bonus is also available. Pay starts at $45 per hour.

Key Responsibilities
The Registered Nurse in Home and Community Based Services is responsible for delivering and overseeing nursing and personal care to patients in their residences, as directed by the physician or authorized by state regulations, ensuring compliance with relevant laws and agency policies. The Registered Nurse collaborates with other healthcare professionals to coordinate care plans and maintain continuity of care by effectively communicating with physicians, agency staff, patients, families, case managers, and community resources.
Specific Duties
- Deliver high-quality clinical services within the scope of practice and infection control standards, in alignment with the care plan and in collaboration with the patient's care team from admission to discharge.
- Conduct clinical nursing assessments as per federal and/or state program requirements and payer specifications.
- Verify patient eligibility and medical necessity for services as outlined by the payer source and agency policies.
- Develop and update individualized care plans in accordance with federal and/or state program requirements in partnership with other community providers.
- Ensure the frequency and duration of care plans meet patient needs and initiate revisions as necessary with physician consent.
Experience Required:
- A minimum of one year of experience as a Registered Nurse is preferred.
- Must possess a current RN license in the state of practice.
- Valid driver's license, vehicle insurance, and access to reliable transportation or public transit.
- Current CPR certification is mandatory.
