Clinical Pharmacist

Job Summary:
The Clinical Pharmacist will provide and coordinate pharmaceutical care services and collaborative drug therapy management for patients. This role serves as a resource to patients, physicians, pharmacists, nurses, and other members of the healthcare team.
Key Responsibilities:
- Assure safe and effective medication use
- Demonstrate proficiency in delivering care to assigned age-specific patient populations
- Participate in related continuing education
Requirements:
- PharmD from an accredited college of pharmacy
- California Pharmacist Registered upon hire
- California Fire and Life Safety Card - National Organization within 30 days of hire
Preferred Qualifications:
- 2 years acute care experience or a pharmacy residency equivalent to 3 years of pharmacy practice experience
