Job Description: The Licensed Practical Nurse (LPN) in Home Health at Ochsner Home Health is responsible for providing quality patient care and technical skilled services in the patient's home. This role involves executing the plan of care as defined by state laws, coordinating with other healthcare professionals, and observing, recording, and reporting patient responses to treatment daily.

In addition to delivering direct patient care, LPNs educate patients' families on proper care and maintenance of a healthy environment, promoting overall patient well-being.

This position requires excellent communication skills, a strong ability to work collaboratively, and a commitment to delivering exceptional patient outcomes.

Required Skills and Qualifications:
  • Current LPN licensure in the state of practice
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
  • CPR Certification required
  • Additional State Specific Requirements: LA – Current LPN Licensure in LA without restrictions, At least one year of experience working as an LPN
