South Ogden Post-Acute is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team in a full-time day-shift position. As an LPN at our skilled nursing facility, you will play a vital role in providing high-quality care to our geriatric patients who require 24/7 attention.
Key Responsibilities:
- Medication Management: Order and administer medications, equipment, and supplies as prescribed by physicians to ensure our patients receive the best possible care.
- Treatment Administration: Prepare and administer treatments and medications as ordered by physicians, taking into account each patient's unique needs and medical history.
- Communication and Advocacy: Serve as a patient advocate, ensuring that other healthcare team members provide care according to each patient's care plan and personal wishes.
- Charting and Documentation: Accurately complete necessary charting and documentation in a timely manner to maintain accurate patient records.
- Continuous Observation and Monitoring: Continuously observe and monitor seriously ill residents, identifying any changes in their condition and notifying physicians and family members as needed.
Requirements:
- Possess a current RN, LPN/LVN license in good standing.
- Effectively communicate with residents, family members, personnel, and visitors.
- Able to make decisions independently when warranted.
- Knowledge of medical and nursing practices and procedures.
- Thorough understanding of local, state, and federal laws and regulations pertaining to long-term care.
