Role Overview

As a House Calls Nurse Practitioner at Abode Care Partners, you will provide personalized medical care to seniors and patients with complex needs in their preferred living environments.

This role involves performing evidence-based medical procedures, collaborating with healthcare professionals, and maintaining comprehensive care plans. You will have the opportunity to grow your medical skills while providing high-quality patient care.


Responsibilities
  • Patient Assessments and Treatment: Perform evidence-based medical assessments and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses.
  • Care Collaboration: Collaborate with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care.
  • Patient Referrals: Provide referrals to other healthcare practitioners when necessary.
  • Medical Record Management: Ensure patient medical records are kept up to date.

Requirements
  • Licensure and Certification: Active state Nurse Practitioners license, Current Nurse Practitioner National Board Certification, and Active state DEA licensure.
  • Education and Experience: Master's degree in Nursing (MSN) and five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
  • Skills: Strong interpersonal communication skills – verbal and written, Excellent time management skills with accountability for practitioner's daily workflow, Proficiency in medical instruments and equipment, and Strong knowledge base of HIPAA and OSHA regulations.

Benefits
  • Competitive Compensation: USD $105,000.00 - $120,000.00 / Year
