We are seeking a compassionate and skilled Nurse Practitioner to join our team at Abode Care Partners. As a Home Healthcare Nurse Practitioner, you will provide personalized medical care to seniors and patients with complex needs in their preferred living environments.

Key Responsibilities:

  • Perform evidence-based medical procedures, collaborating with healthcare professionals to maintain comprehensive care plans">
  • Provide personalized medical care to seniors and patients with complex needs">
  • Collaborate with caregivers and other healthcare professionals in providing interdisciplinary care">
  • Order and interpret laboratory and diagnostic testing as needed">
  • Ensure patient medical records are kept up to date">

Qualifications:

  • ">
  • Active state Nurse Practitioners license">
  • Current Nurse Practitioner National Board Certification">
  • Active state DEA licensure">
  • Master's degree in Nursing (MSN)">
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred">
  • Strong interpersonal communication skills – verbal and written">
  • Excellent time management skills with accountability for practitioner's daily workflow">
