Job Title: Physical Therapist - Home Health Visits

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in providing high-quality home health care services to our clients in Holmes, Pennsylvania.

About the Role:
  • As a Physical Therapist, you will be responsible for assessing and treating clients in their homes, providing individualized care plans to promote their health and independence.
  • You will work closely with our multidisciplinary team to develop and implement care plans, educate clients and their families on rehabilitative care, and document observations and interventions.
  • Our Physical Therapists are passionate about delivering exceptional patient care and making a meaningful difference in the lives of our clients.
Responsibilities:
  • Perform diagnostic tests and measurements to assess client needs and develop individualized care plans.
  • Develop and implement manual therapeutic exercises, gait training, balance, and other interventions to promote client health and independence.
  • Continually assess and revise care plans to ensure optimal client outcomes.
  • Provide education and instruction to clients and their families on rehabilitative care and activities necessary for independent living.
  • Accurately document observations, interventions, and evaluations to ensure seamless communication with the care team.
Qualifications:
  • A current Pennsylvania Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Solid computer skills and prior experience with electronic medical records (EMR) are preferred.
