About the Role

We are seeking a highly skilled Physical Therapist to join our team at Andwell Health Partners. As a Physical Therapist, you will play a vital role in providing high-quality, patient-centered care to individuals in their homes and communities.

Key Responsibilities
  • Provide physical therapy services to clients according to the standards of practice outlined by the American Physical Therapy Association.
  • Develop, implement, and revise individualized plans of care that incorporate goals consistent with the client's age, condition, and needs.
  • Collaborate with clients, families, physicians, and other healthcare providers to coordinate care and promote client independence and well-being.
  • Implement client care plan activities based on treatment goals to support independent living.
Requirements
  • Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited institution.
  • Current PT license in the State of Maine.
  • Current CPR certification.
  • 1-year experience in physical therapy, with home health experience preferred.
  • A reliable vehicle with insurance.
