Job Summary:
The Social Worker is a vital member of the WakeMed team, responsible for providing exceptional care to patients and families affected by medical, psychological, and social challenges. This role requires a compassionate and skilled professional who can navigate complex situations and ensure timely discharge and optimal outcomes.
Key Responsibilities:
- Coordinate patient-specific needs, including placement, basic living resources, home health resources, and post-hospital support.
- Collaborate with the Discharge Planner and other patient care team members to ensure seamless care transitions.
- Advocate for patients' physical, psychological, and social needs, ensuring their rights are respected and addressed.
- Develop and implement effective plans to address patients' social and emotional challenges.
Requirements:
- 1 year of experience in clinical mental health or substance abuse/addiction required.
- Bachelor's Degree in Social Work required.
Work Schedule:
Monday to Friday, 8:00 AM to 4:30 PM. No weekends or call requirements.
