Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Intake team as a Patient Transition and Referral Specialist. This role will be responsible for facilitating the transition of patients from skilled nursing facilities and physician's offices to home care services.

Key Responsibilities
  • Provide education and information regarding VNA programs, policies, and services to the local community, healthcare facilities, physicians, patients, and their families.
  • Work closely with skilled nursing facilities, physician's offices, the community, and VNA staff to facilitate patient transition to home care.
  • Maintain and develop contacts and knowledge of local community resources.
  • Utilize knowledge and skills to convert informational calls into home care referrals.
  • Meet monthly budget numbers and maintain accurate records of patient information.
  • Answer incoming calls in a timely manner and effectively manage calls from external referral sources.
  • Identify unmet learning needs and seek assistance from others in meeting those needs.
  • Comply with company policies and procedures.
  • Complete all required forms, reports, time cards, itineraries, etc., accurately and within designated timeframes.
  • Recognize and follow established lines of communication.
Requirements
  • High School Diploma or GED.
  • Graduate of an accredited LPN program.
  • Associate's degree in Nursing preferred.
  • 1 year hospital experience, preferred.
  • Home care and/or Community Health experience is preferred.
  • Experience working as part of a team and working effectively with persons and communities from diverse cultural, social, and ethnic backgrounds.
  • The ability to work in a complex institutional structure requiring significant collaboration.
  • Ability to work independently with minimal supervision.
  • Demonstrated ability to maintain strict confidentiality.
