Mental Health Systems, Inc. is seeking a highly skilled Care Coordinator to join our team. As a Care Coordinator, you will play a critical role in providing comprehensive care to individuals with mental illness and substance use disorders.
Key Responsibilities- Case Management: Provide case management services to clients, including crisis intervention, discharge planning, group and individual work, mental health and substance use disorder counseling, screening, and assistance with basic needs and symptom management.
- Documentation: Complete accurate and timely documentation, meeting minimum billable requirements for direct client services.
- Facilitation: Facilitate placement, including pre-placement visits and transportation as needed.
- Shift Coverage: Provide shift coverage, including part of the on-call team.
- Field-Based Services: Provide field-based services as required by program and client need.
- Confidentiality: Maintain confidentiality of client care.
- Meetings: Participate in scheduled program meetings.
- Education: Bachelor's degree in a behavioral science and at least four years of experience providing services to individuals with mental illness.
- Experience: Three years of experience providing case management to individuals with mental illness and/or substance use disorders preferred.
- License: Valid CA driver's license and current auto insurance per contract requirements required.
- Cultural Competency: Cultural competency understanding required.
- Language: Bilingual preferred.
- Software: Knowledge of Microsoft Office and EHR software preferred.
- Seeing: Ability to see.
- Hearing: Ability to hear.
- Speaking: Ability to speak.
- Stooping/Bending: Ability to stoop and bend.
- Working in cramped spaces: Ability to work in cramped spaces.
- Moving around facility: Ability to move around the facility.
- Moving between offices/clients: Ability to move between offices and clients.
- Driving: Ability to drive.
- Climbing: Ability to climb.
- Lifting/carrying heavy items: Ability to lift and carry heavy items.
- Pushing/pulling/dragging items: Ability to push, pull, and drag items.
