EAC Network, a leading not-for-profit social service agency, is seeking a dedicated Health Home Plus Care Coordinator to join our Nassau County Long Island Health Home Plus Program.

About the Role

As a Health Home Plus Care Coordinator, you will play a vital role in transforming the lives of individuals with severe mental illness and related challenges. You will coordinate care, conduct outreach and assessments, and develop service plans to ensure clients receive comprehensive support.

Key Responsibilities
  • Lead the way in coordinating care for clients with severe mental illness, substance use disorders, and chronic medical conditions.
  • Conduct outreach, assessments, and service planning to help clients achieve their health goals and maintain contact with their support networks.
  • Engage in home visits, community work, and hospital visits to ensure clients are connected with the services they need.
  • Collaborate with a multidisciplinary team to create and implement effective care plans.
  • Monitor and support client progress, assist with crisis interventions, and ensure follow-up on medical directions and aftercare discharge.
Requirements

We are looking for a candidate with a Master's degree in a human services field and one year of experience, or a bachelor's degree with two years of experience, or a CASAC with two years of experience. Alternative qualifications include three years of experience in a relevant field or two years as a Health Home care manager.

Direct service experience with serious mental illness, developmental disabilities, substance abuse, or children with serious emotional disturbance is required. Excellent organizational, time management, and communication skills are necessary. Proficiency in Microsoft Office and other relevant software is also required.
