Overview: Sentara Regional Resource Pool is seeking an Inpatient Case Manager, MSW to join our dedicated team. This role involves providing essential support across multiple hospitals in the region, with assignments determined by patient care needs.
- Operational Hours: 8:00 AM – 4:30 PM, available 7 days a week
- Flexibility: Team members are expected to commit to a minimum of 48 hours monthly, including 16 hours on weekends
- Certification: Candidates must obtain Case Management certification (CCM, CMAC, or ACM-SW) within one year of eligibility
- Education: A Master's degree in Social Work is required
- Experience: At least one year of relevant experience is necessary
- BLS Certification: Must be obtained within 90 days of employment
As an Inpatient Case Manager, your key responsibilities will include delivering medical social work services to patients, evaluating their needs, collaborating with healthcare professionals and community resources, and crafting effective discharge plans.
