The successful Case Manager will possess excellent communication skills and the ability to work effectively with patients, families, and medical teams. They will be responsible for assessing, planning, implementing, monitoring, and evaluating actions required to meet the client's health and human services needs.
A key aspect of this role will be to develop effective working relationships with medical teams and cooperate throughout the entire case management process. You will also be responsible for facilitating multiple care aspects, including case coordination, information sharing, and arranging abeyances with social services, health, and governmental agencies.
The Case Manager will promote quality and cost-effective interventions and outcomes while adhering to professional standards outlined by protocols, rules, and regulations. A certified Case Manager is a plus, but not required.
Responsibilities:
- Coordinating and providing care that meets the unique needs of patients.
- Handling case assignments, drafting service plans, reviewing case progress, and determining case closure.
- Facilitating multiple care aspects, including case coordination, information sharing, and arranging abeyances.
- Helping patients make informed decisions by acting as their advocate regarding clinical status and treatment options.
- Developing effective working relationships with medical teams and cooperating throughout the entire case management process.
