* Evaluate behavioral, emotional, cognitive, spiritual, and social needs of clients
* Develop and implement personalized treatment plans
* Facilitate individual and family sessions, therapeutic groups, and referral source/outpatient provider contact
* Provide detailed and personalized aftercare planning and community outreach resources/referrals
* Other duties as assigned
