Job Opportunity at Reliant - Berlin Rehab and Healthcare Center - NSS

We are seeking an experienced Physical Therapist to join our team at Reliant - Berlin Rehab and Healthcare Center - NSS. As a Physical Therapist, you will play a vital role in managing treatment plans to promote positive outcomes for patients with diverse diagnoses in a skilled nursing facility (SNF) setting.

Key Responsibilities:

  • Develop and implement individualized treatment plans to meet the unique needs of patients
  • Conduct thorough assessments and evaluations to identify patient goals and objectives
  • Collaborate with interdisciplinary teams to ensure comprehensive care
  • Provide education and training to patients and their families on proper techniques and exercises

Requirements:

  • Current BLS certification (AHA preferred)
  • Active state license to practice as a Physical Therapist
  • Supervisory professional references
  • Must complete drug screen and background screen
