Key Responsibilities- Collaborate with healthcare teams to develop and implement individualized care plans
- Conduct thorough assessments of patient needs and develop plans to address them
- Coordinate with community resources to ensure patients receive necessary support and services
- Communicate effectively with patients, families, and healthcare teams to ensure smooth transitions
- Monitor patient progress and adjust care plans as needed
- Registered Nurse licensure in Illinois
- 2-3 years of clinical nursing experience
- CPR certification
- Ability to work in a fast-paced environment and prioritize multiple tasks
- BSN degree
- ACM or CCM certification
- Knowledge of insurance guidelines and CMS requirements
