Job Description

**Job Summary:**

We are seeking a highly skilled and experienced Registered Nurse to join our team as a RN Supervisor at Sanzie Healthcare Services, Inc. The successful candidate will be responsible for providing skilled nursing care and supervision to clients in their homes, ensuring the highest level of quality care and patient satisfaction.

Key Responsibilities:

  • **Client Assessment and Care Planning:** Conduct initial face-to-face assessments of clients in their homes, establish and implement individualized care plans, and provide ongoing assessments and revisions as needed.
  • **Supervision and Education:** Supervise and educate Certified Nursing Assistants (CNAs) and other home health aides, ensuring they provide high-quality care and adhere to Sanzie Healthcare Services, Inc.'s policies and procedures.
  • **Clinical Documentation:** Maintain accurate and comprehensive clinical records, including progress notes, care plans, and other relevant documentation.
  • **Communication and Collaboration:** Communicate effectively with clients, families, and healthcare team members to ensure seamless care coordination and patient satisfaction.
  • **Quality Improvement:** Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance client care and outcomes.
  • **Compliance and Regulatory:** Ensure compliance with all relevant laws, regulations, and accreditation standards, maintaining the highest level of professionalism and integrity.

Requirements:

  • **Education:** Current RN licensure and a minimum of 2 years of experience in home health or a related field.
  • **Skills:** Strong assessment and care planning skills, excellent communication and interpersonal skills, and the ability to work independently and as part of a team.
  • **Certifications:** Current CPR certification and any other relevant certifications or training.

Compensation:

$40 - $50 per visit
